Transportation is one of the biggest barriers for our patients. This is why bus cards are always needed and very appreciated. Greater Richmond Transit Company (GRTC) has a fare-card system with values of $5 and $10. Cards are available at many local CVS and Kroger stores. Diabetes can effect multiple systems within the body. Unmanaged diabetes can lead to heart disease, gum disease, neuropathy, blindness, and amputations. Daily Planet Health Services offers a diabetes impact program for patients who face barriers to improved health. Through our shared care model patients are seen by their primary care provider and pharmacist during the same appointment, in addition to weekly health education and a exercise program. Here is a list of items that our patients and providers have identified can help with diabetes management: nail files, nail clippers, pumice stones, hand mirrors, Vaseline, cocoa butter, water bottles, hula hoops, and bus cards.
We no longer take clothing donations. We do not have the storage space or needed resources to operate a clothing closet.
