Mobile Outreach
Daily Planet Health Services has two mobile units that provide care throughout Greater Richmond directly in the communities where patients live. The program allows DPHS to reach members of our community who are homeless, housing insecure, and residents of public housing.
The mobile outreach program provides:
- Comprehensive care and treatment for all ages
- Treatment for chronic conditions including diabetes, high blood pressure, HIV, Hepatitis C, and heart disease
- Preventative screenings including labs and vaccines
- Case management and outreach services
Patients receiving care through the mobile medical outreach program must meet criteria for becoming a new patient of Daily Planet Health Services.
